National AIDS Trust Trading Questionnaire 2007

As a valued supporter of the National AIDS Trust through the purchase of HIV awareness-raising materials, your opinion of the materials and the service we provide means a great deal to us. With your help we can ensure we supply the correct materials, maintain the service you require, and continue to support your essential work effectively.

Please take ten minutes to complete the following questionnaire, to let us know your requirements and help us develop the materials you need.

As an added thank you we will select at random one respondent who will receive a £50 voucher for their choice of Marks and Spencer, Boots, Argos, John Lewis or Virgin Megastore, or £50 off their next National AIDS Trust purchase.

Terms And Conditions – Gift voucher Draw

  1. The one selected respondent will receive a gift voucher of £50 value for their choice of one of the following retailers: Marks and Spencer, Boots, Argos, John Lewis, Virgin Megastore, or £50 off their next National AIDS Trust purchase.
  2. The one selected respondent will be chosen on 15 May 2007 and contacted via the contact details provided.
  3. One entry per person only.
  4. No purchase necessary, to enter please fill in the questionnaire and submit to the National AIDS Trust as instructed.
  5. Entries should arrive no later than 11 May 2007, no entries will be accepted into the draw after this date.
  6. No cash alternative is available in place of any prizes and the Promoter reserves the right to substitute the prize for another of equal value if necessary. The prize is non-transferable.
  7. The selected respondent will not have their details advertised, but that information will be available on written request.
  8. The judges' decision is final and legally binding and no correspondence will be entered into.
